The form is sent automatically five days prior to your appointment via email and/or text message. You will want to fill this out as honestly as possible to guarantee the best service. If you have not received your consultation form, please call us at 843-875-4588 and we will be happy to send one to you.
If you are a new guest, you may book a complimentary "New Client Introduction" or "Consultation" with your desired service or as a stand-alone consultation appointment.
